Vehicle Registration Forms

1/2 and 3/4 Ton Truck Owner's Weight Certification (Form #VR-142A) is used when an owner of a 1/2 or 3/4 ton truck requests to carry a gross vehicle weight of 10,000 pounds (instead of 7,000 lbs). 1/2 ton trucks must be modified to carry the increased weight and 3/4 ton trucks must have a shipping weight of the chassis and body of more than 1,500 pounds or rated by the manufacturer at a gross vehicle weight in excess of 7,000 pounds. This form can be brought to any full service MVA office.

Address Certification for U.S. Government Employees (Form #VR-102) can be used by U.S. Government Employees to certify their address when out-of-state. This form can be filled out and brought in person to your local full service MVA office or mailed to the Motor Vehicle Administration, 6601 Ritchie Hwy., Glen Burnie, MD 21062, Attn: Title Correspondence.

Affidavit of Lawful Possession for Transfer of a Vehicle to an Automotive Dismantler and Recycler or Scrap Processor (Form #VR-454) is used to certify, under oath, that a title is not available for a vehicle that is being transferred to an A D& R or Scrap Processor for purposes of dismantling, destroying or scrapping.

Affidavit in Lieu of a Title​ (Form #VR-449A) is to be used by companies who have created a new titling record and are in need of a duplicate title for the previous owner. When using this form you will need to include a copy of the owner/co-owner state issued identification card.

Affidavit of Ownership for Transfer of a Vehicle to an Automotive Dismantler and Recycler or Scrap Processor (Form #VR-453) is used by a vehicle owner, or their agent, to certify, under oath that a title is not available when transferring the vehicle to an AD&R or Scrap Processor for dismantling, destroying or scrapping.

Affidavit Manufactured Home Converted to Real Property (Form #VR-451) is to be used when a manufactured home is being converted to real property.

Affidavit Manufactured Home Severed From Real Property (Form #VR-452) is to be used to certify under oath a manufactured home has been severed from real property.

Affidavit of Ownership - Moped, Motor Scooter & Off Road Vehicle (Form #VR-450) document is used by an applicant who does not have a title or certificate of origin for their moped, motor scooter or off road vehicle. The form takes the place of the ownership document, the decal application and the application for title for these transactions.

Application for Approval of Emergency/Non-Emergency or Service Vehicles (Form #VR-026) is to used apply for approval to be a designated vehicle that is recognized as an Emergency or Non-Emergency Vehicles. The form can be returned either in person or by mail to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 104, Glen Burnie, MD 21062.

Application/Certification for Issuance of Farm Tags (Form #VR-097) is used by customers to certify that both the vehicle and the owner meet the qualifications for Farm Registration.

Application for Assigned Vehicle Identification Number (Form #VR-198) is used by customers who make homemade vehicles, certain kit vehicles, and for lost VINS.

Application for a Certificate of Title (Form #VR-005) can be used for titling a vehicle. This form must be filled out and brought in person to your local full service MVA office.

Application for a Corrected Title Due to a Name Change (Form #VR-448) is used to apply for a corrected title for a name change.

Application for Duplicate Certificate of Title (Form #VR-018) can be used to apply for a duplicate title for a vehicle. This form must be filled out and can be brought in person to your local full service MVA office or mailed to the Motor Vehicle Administration, 6601 Ritchie Highway, N.E., Glen Burnie, MD 21062, Attn: Title File, Room 104.

Application for Duplicate or Substitute Plates/Stickers (Form #VR-009) This form must be filled out and can be either brought in person to your local full-service MVA office or it can be mailed along with the appropriate fees to the Maryland Motor Vehicle Administration, 6601 Ritchie Hwy, Attn. Registration Renewal Section, Glen Burnie, MD 21062.

Application for Duplicate Security Interest Filing (Form #VR-048) is used to request a duplicate security interest filing. The completed form, along with the appropriate fees, can be brought to any full service MVA or mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 4, Glen Burnie, MD 21062.

Application for Historic or Street Rod Registration (Form #VR-096) is used to apply for historic or street rod license plates. You will need to fill out the form and provide all necessary documentation. The form can be brought to any full service MVA location or mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, Maryland 21062, Attn. Vehicle Registration, along with the appropriate fees.

Application for Issuance of Farm Area Vehicle Tags or Island Tags (Form #VR-331) is used by customers to certify they meet the qualifications for Farm Area / Island Vehicle Registration. The completed form along with any necessary documentation can be brought to any full service MVA location or mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, Maryland 21062, Attn. Vehicle Registration, along with the appropriate fees.

Application for License Plates for Facilities for License Plates for Facilities (Form #VR-059) is used for an owner or operator of a nursing home, health care facility, adult day care, retirement home, etc. to apply for disability license plates for vehicles owned by the facility where they provide transportation for individuals with disabilities.

Application for Lien Code Assignment and/or Enrollment/Change in Electronic Lien System (Form #VR-466) This forms allows for Lien Code requests and/or Electronic Lien Service requests.

Application for Maryland County Pride Stickers (Form #VR-308) is used to apply for the Maryland County Pride Stickers. This form can be filled out and brought in person to your local full service MVA office along with the appropriate fee.

Application for Maryland Parking Placards/License Plates for Individuals with a Disability (Form #VR-210) is used for individuals with a disability to apply for a parking placard or license plate. You will need to fill out this form, and your physician will need to complete the disability certification portion of the application. The completed form can either be brought in person to a full-service MVA office or mailed to the MVA, Disability Unit, 6601 Ritchie Highway, Glen Burnie, MD 21062.

Application for Military Related License Plates (Form #VR-120) is used to apply for a military related license plate. The form can be returned either in person or by mail, along with the appropriate fee and documentation, to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 104, Glen Burnie, MD 21062.

Application for New Plates/Stickers & Transfer of Plates or Non-Title Trailers (Form #VR-008) can be used to apply for new plates and stickers or to transfer plates or non-title trailers. This form can be filled out and brought in person to your local full service MVA office or it can be mailed to Maryland Motor Vehicle Administration, 6601 Ritchie Hwy., Glen Burnie, MD 21062, Attn: Vehicle Registration, along with the appropriate fees.

Application for Nonresident Permit (Form #VR-111) may be brought to any full service MVA for processing when completed. It can be mailed in to the MVA with the required fee to 6601 Ritchie Hwy.NE, Room 104, Glen Burnie, MD 21062. The form may also be taken to a licensed Tag and Title Service. Students at Towson University may submit the form to the Parking Services office, University of MD students may submit the form to the Municipal Center office.​

Application for Registration Authorization Farm Area Vehicles (Form #VR-325) is used for out-of-state individuals or a company to apply for a temporary registration while under contract with a Maryland farmer to conduct seasonal harvesting operations. The form must be completed and submitted with a level one inspection certificate and proof of insurance to the Salisbury Branch Office or Talbot County Branch Office or mailed directly to the Maryland Motor Vehicle Administration, Motor Carrier Services Section, Room 120, 6601 Ritchie Highway, Glen Burnie, MD 21062.

Application for Registration Plate Refund (Form #VR-021) can be used to apply for a refund for registration plates. Please follow the instructions on the form. This form can be filled out and brought in person to your local full service MVA or it can be mailed to Maryland Motor Vehicle Administration, 6601 Ritchie Hwy., NE, Glen Burnie, MD 21062, Attn: Tag Return.

Application for Salvage Certificate/Owner Retention (Form #VR-028) may be used by an insurance company who acquires a vehicle as a result of a claims settlement, an Automotive Recycler or Dismantler that acquires a salvage vehicle from a source other than an insurance company, a vehicle has been acquired as an abandoned vehicle from a police auction, or by any other person who acquires or retains ownership of a vehicle that is salvaged. Insurance companies and individuals requesting a Salvage Certificate must submit, with this application, the Title certificate for the vehicle. This application may be brought in person or mailed to the MVA, 6601 Ritchie Hwy, Glen Burnie MD 21062, Attn Salvage Unit, Room 104.

Application for Self-Service Storage Facility Lien (Form #VR-472) is used by an operator, owner, lessor or sup-lessor of a self service storage facility that has a signed rental contract and the renter is more than 60 days in default of their rental agreement.

Application for Exemption from Estate Administration to Transfer a Decedent's Vehicle (Form # VR-481) is used when a vehicle owner has passed.  The surviving spouse certifies no more than two vehicles were owned by the decedent upon death and they are the only heir or legatee. For additional information about the criteria that must be met please refer to the application.

Application for Special Registration Plates (Form #VR-164) is used to apply for personalized (vanity) plates or plates for amateur radio operator registration. The completed form can be mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, MD 21062, Attn: Vehicle Registration along with the appropriate fees.

Application for the Use of Vintage Registration (Form #VR-188) is a form used by the owner of a registered historic vehicle, requesting display of an authentic Maryland license plate issued in the same year as the historic vehicle’s model year.

Autocycle Certification (Form #VR-339) is used to certify a vehicle meets all of the requirements under Maryland Law to be titled and registered as an autocycle.

Business Entity Submission of Proof for Vehicle Titling (Form #VR-475) - please use this form if a Maryland title is being presented to retitle a vehicle for use with a business and new Z identification number is being requested.

Certification for Multipurpose Passenger Vehicle Registration (Form #VR-095) is used for customer to certify that vehicles originally manufactured as trucks, now meet the qualifications for Multipurpose Passenger Vehicle and indicating the additional equipment that makes the class change possible.

Certification for the Issuance of Dump Service License Plates as Provided by the Maryland Vehicle Laws (Form #VR-098) used by customers applying for Dump Truck registration to certify that the vehicle meets all of the requirements under Maryland Law for this type of registration.​

Commemorative Bay Plate / Agricultural Plate Application (Form #VR-302) contains information on the Commemorative Chesapeake Bay Plate and the Agricultural Plate as well as an application form.

Excise Tax Credit Request for Plug-in Electric Vehicle (Form VR-334) is used by customers applying for the excise tax credit allowed for plug-in electric vehicles. The form describes the qualifications and has a place for the vehicle owner to certify they meet the qualifications for the excise tax credit.

Excise Tax Credit Request for Plug-in Electric Vehicle (Spanish Version)​ (Form VR-334S) is used by customers applying for the excise tax credit allowed for plug-in electric vehicles. The form describes the qualifications and has a place for the vehicle owner to certify they meet the qualifications for the excise tax credit.​

Flagging Program Jurisdictions and Locations (Form #VR-143) - is a list of the Parking, Red Light, Tolls and Speed Camera jurisdictions. Each jurisdiction is listed by their agency's four digit code, their telephone number and location.

Gift Certificate (Form #VR-103) can be used for titling a vehicle if it is to be given as a gift from one family member to another family member. This form must be completed and submitted with the assigned title. If transferring a vehicle from an Aunt or Uncle to a Niece and/or Nephew you will need to have the Certified Statement (Form #VR-299) accompany the Gift Certification form.

Insurance Affidavit in Lieu of a Title (Form #VR-449) is to be used by insurance companies who have settled total loss claims in making application for a salvage certificate, when the title has become lost, defective or destroyed.

Limited Speed Vehicle Certification (Form #VR-338) describes a limited speed vehicle; provides driving restrictions for limited speed vehicles; emblem display requirements for limited speed vehicles; and a certification for both the dealer and vehicle owner to indicate their understanding of these requirements.

Low Speed Vehicle Application (Form #VR-324) is used when a four-wheeled electric vehicle that has a maximum speed capability that exceeds 20 miles per hour but is less than 25 miles per hour is titled and registered in the State of Maryland. Maryland Dealer Reassignment (Form #VR-182) is used by a dealer to assign ownership of a vehicle.

Motorcycle Equipment Certification (Form #VR-094) is used by customers and dealers to certify that the motorcycle meets requirements for registration and also informs customers of restrictions of motorcycle use depending on the engine size of the vehicle.

Odometer Disclosure Statement (Form #VR-197) can be used by a license dealer in the State of Maryland. Allows the disclosure of a vehicle's odometer mileage from the transferor (seller) to the transferee (buyer). This form should accompany the title transaction.

Off-Road Recreational Vehicle Permit Application (Form #VR-337) is to be used by customers to apply for the Off_Road Vehicle Titling Sticker that is placed on the rear of the off road vehicles. This sticker may be issued to current or new owners of Off Road Vehicles and is for vehicle and owner identification purposes only. This sticker does not allow the vehicle to be used on roadways or authorized DNR trails.

Schedule of Fees (Form #VR-042A) used by dealers, customers and the MVA to determine the correct fees, both full year and half year for larger trucks, tractors, dump trucks, etc.

Request for Exemption of Excise Tax and Title Fee for Qualifying Vehicle Transfers to or from a Trust (Form #VR-478) To apply for title fee and excise tax exemption for transfers into and out of trusts which are excise tax exempt under 13-810 (c) 26, and Estates and Trust Annotated Code of Maryland §14.5-1001.

Security Interest Filing Statement (Form #VR-217) can be used to record a lien on an existing title certificate or for the original lien holder to reassign the security interest in a vehicle. The Certificate of Title must accompany this form. Taxicab Certification (Form #VR-293) This form is used by the vehicle owner to certify that the vehicle meets all standards to be registered as a taxicab.

Temporary Inspection Waiver (Form #VR-129) can be filled out and brought in person to your local full service MVA or it can be mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 30, Glen Burnie, MD 21062.

Tow Truck Certification (Form #VR-294) is used for customers to certify that all conditions have been met for them to register their vehicle as a Tow Truck.
